Collaboration for Psychological Wellness, LLC believes in providing access to mental health services and therefore we are currently an in-network (INN) provider for clients with the following insurances: Blue Cross and Blue Shield; HealthPartners; Minnesota Health Care Programs (including MinnesotaCare & Medical Assistance); Preferred One/Aetna, Optum (including UnitedHealthcare, Medica, & UMR) and UCare. If you would like to use insurance benefits for your services, please let us know while requesting your first appointment and we will work together to verify your benefits prior to starting services. However, please note that not all of our clinicians are credentialed with all of the above-mentioned insurance companies.
Collaboration for Psychological Wellness, LLC is an out-of-network (OON) provider for all other insurance companies at this time, but is actively working on getting in-network with the other major companies in the area in order to be of service to more members of the community. However, if you would like to pursue insurance reimbursement for out-of-network services, please verify this benefit with your insurance company and then connect with Collaboration for Psychological Wellness, LLC to discuss options for the process prior to scheduling services.
We also can also discuss private pay options if you would like to proceed with services without the use of insurance benefits.
Private pay fees for mental health services at Collaboration for Psychological Wellness, LLC currently range between $125.00 to $250.00 per session depending on type of service.

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Life happens and situations understandably arise in which you will need to cancel or reschedule your appointment. If you need to cancel your appointment, please do so at least 24 hours prior to your scheduled appointment.
Appointments that are not cancelled within that timeframe or that are missed will be charged the Late Cancellation/Missed Appointment Fee of $100.00.
Payments are due at the time of the service. Payments may be made electronically through debit card, credit card, or prepaid credit cards (HSA). Please note that payments for telehealth sessions are also due at time of service and will need to be made via debit card, credit card, or prepaid credit cards.
All clients are required to keep a credit card on file and credit card information must be provided at the time of the first appointment. Credit card information is securely stored within our credit card processing system using HIPAA-compliant and PCI-compliant encryption.
